Change of Address (form 5300.38)

Has anyone filled out one of these?

Only box 16 is marked "Not applicable for collectors", when
clearly many of the boxes are not applicable for C&R FFLs.

The instructions mention nothing about which boxes are
N/A for collectors and there is nothing on the BATFE
website about it.

Also, box 20 on this form asks for a copy of ones lease if you are a renter.
I didn't have to provide a copy of my lease to apply for the FFL,
why would I now have to provide one to do a change of address?

Before I call BATFE, I just wanted to check if anyone here had done
a change of address and what boxes (if any) they ignored.

I wouldn't worry to much about it, as long as you have your license number, your old adress and your new adress on it they will take care of it. Oh and fax it in instead of mailing it, I faxed mine and had my new license in 2 weeks.
