C&R app question

I applied for a C and R a couple years ago but the put down a wrong address and never received it. I called them, when this happened, and they said I would have to fill paperwork to fix it. I never did and just forgot about it.

I have since moved and was wondering if I just apply for a new one or have to amend the one on file.

Thanks, I have no clue what happened to it. I 'll just go through the steps and hope nothing comes up.
