FFL 3 form confusion

I asked for an FFL3 application from my local small town BATF office some months ago and he sent me 2 copies. I filled one in, sent it in and never heard from it again.

A few weeks ago I sent in the 2nd copy. I hand carried a copy of it to my police department and cleared it with them. I got a post office money order, no problems with a credit card or anything if that was the case.

Just got it back. It was returned to sender. I checked the address on my form and it should have been OK.

Then I checked the form and it does NOT match the form on the sticky pdf above. It is a Department of the Treasury Bureau of A.T.F. Application for Licence (Collector of Curio and Relics) and the mailing address listed is:

Bureau of Alcohol, Tobacco and Firearms
PO Box 845219
Dallas, Texas 75284-5219.

What is up? Slightly different form to the one above; different address and state to be sent to. Am I failing an IQ test?

That sticky needs to go away, since ATF has their own PDF of the form now.

Just compared my form again. The OMB number is 1512-0518 and the one on the sticky is 1140-0038. I DID get it from my local small town BATF.

Is mine obsolete, acceptable, newer, a figment of my BATF office's imagination?

sent it to the wrong place. send it to the atlanta address on the back of the form at the top.

go to the site, order the FFL Green forms, it's on the applications link i believe. Have them send it to you directly. The Money order goes to the ATF (not local pd) along with the copy that says ATF on the bottom in red. The second copy should have the payment section blanked out and read on the bottom in red CLEO which should be addressed to the chief of police.

Hope that helps! the ATF sent me the form in one week, although i heard stories where people get them and it takes almost a month!

Well since the form they sent me does look different and have a different code, I printed out the newer one and sent it off with my money order. Hope a PDF is OK; I can't get anything modern out of my town I am sure.

Sent a new copy to my CLEO with an explanation.

I should have had this in November, now I know - my office sent me obsolete forms. Posting this in case someone does a search sometime.

The form is now downloadable, and you can even complete the application on your PC and print it, sign it, and mail it to the ATF and a copy to your CLEO.

With the address in Dallas, that means the first form you got was about 10 years old. They used to forward the applications from Dallas to Atlanta for a few years after the move, but now they just return them to sender.
