    GOT MY C&R application form.

    Now what.
Firstly do I have to fill out every page?

Ssecondly where do I send it, with what else and how much does it cost. All I got was on green form in an envelope and that was my lot.

I already got my COE applictaion form. not sure what to do with that either. I'm in San Francisco - anyone else done this round this neighborhood?

Yes, you have to fill out every page. The address is on the top of the instructions page. The only place you should get confused is item 10. Just write "US Citizen" in big letters then "N/A" all the fill-ins for item 10. Wherever it asks for your title --put "Collector".

For the COE, fill out the PDF form and print it. Find a livescan fingerprint operator -- look online. Everyone from school employees to security guards need to be fingerprinted by livescan. I found a local place that does income taxes, passport photos, a notary public and does livescan. Our local junior college does livescan. You can do the whole thing in a couple of hours and send it off. BTW -- check "Collector" on the COE form.

I sent my COE app by USPS priority mail and had it in 3 weeks.

Questions, PM me and I'm sure I can talk you through it.
