Bound Book template

Anyone have a bound book template that they found worked best for them?

Word doc or excel xls format?

I scanned this http://www.surplusrifle.com/graphics/boundbookpage.pdf into a doc file and reformatted some lines, etc. I print a updated copy after each purchase for my bound book (3-ring binder).

Not to get to far off topic but I took surplusrifle's page and made about 25 copies. Then I went down to staples, they sent the pages away and actually "bound" them in a real book, the coolest thing about it was it only cost $12.00!
