    Hi, returning the favor for board members. I used the PDF versions from here, and followed up with the pre-filled green form they sent me. Now that I have my C&R, they *finally* got around to sending me the forms from my original request (over 3 months ago!)

    I have two original C&R applications (green form) up for grabs that I'll send out complimentary. First two replies on this thread gets them. PM me your address.
