Need a C&R Application...

Anyone got a scanned copy that can be used? The online versions I've seen are old and have the old application address listed. I've requested a new app from the ATFE's website but this was about 4 weeks ago and still nothing has shown up in the mail. Ugh! If no one has a scanned copy, I guess I'll wait.. Once I get it, I'll scan it and make it available for everyone else to use.
