Help with form for Ventura County

Anyone know the name and address of the Chief Law Enforcement Officer for Ventura County (Thousand Oaks). Actually, I'm not sure if it's for the County or the City....help

Thousand Oaks uses the Ventura County Sheriff's Department for a police force. The Chief of Police for Thousand Oaks is also a Commander in the VCSD...
