That's a new one factory or reload ammo?

Wow....talented. Any more details on the incident? Ammo brand? First shot? 100th?

I was warned about this while reloading but never heard of it happening on factory ammo.

Well it was the last stage in a 6 stage rifle match, probably more than 150 rounds. It had been acting up all day, 12" entry gun. Gas pressure is too high during the unlocking cycle which causes primers to blow out. Usually they fall away, but this one decided to get in just the right place.

I had a batch of Federal XM193 that had a nasty habit of blowing the primers out; reported the lot number but never heard back.

Would a heavier BCG (if you don't already have an auto) or heavier buffer weight help with this?

How does one address the high gas pressures?
