Had a Malfunction at the Range

So I was shooting my full sized M&P 45 at the range today and the last round in the magazine... detonated ? The recoil felt very different, and extremely harsh.

I had powder burns on my hands, and the magazine follower was forced down into the magazine and cocked over. Originally I thought the follower was cracked and broken.

When I disassembled there were two small brass nuggets that fell out, most likely from the magazine well area.

Inspected and cleaned. Function checked, and later shot it again with no problem.


I think, and this is just me thinking, that I probably loaded a round that has a bullet pushed significantly back into the case. Does that sound reasonable to the other people here?

Well you probably had a double charge or a case failure.

Bullet setback could cause what you are describing since that would increase case pressure from the decreased internal volume of the case.

It is generally not a good idea to chamber and rack bullets out of the gun more than once since it could cause the bullet to move in the cartridge.

You might want to send it in to the factory to have them verify there are no cracks in the frame, slide, or barrel.

Did it fire out of battery?

Where did the ammo come from? If it went off before fully chambering, which is sure sounds like it did, I'd suspect a high primer struck in the process of the round being picked up by the slide.

Generally if a round goes into my chamber, it's going down range shortly after.

Do you use factory ammo or reload ?

I'll use LAX Ammo for a class, but generally use American Eagle at the range since I have a large reserve of it

Were you able to retrieve the brass from that round? Or did it get lost in the already large pile of range brass?

We looked for about 5 minutes, but there was no way we were going to find it.

It's an all steel plate range with an "always hot" line. Probably 600 pcs of brass on the ground at that point from just our group.

I would think pretty rare but it does happen. Know of a guy who blew up his brand new HK on its first outing with factory ammo. Gun was destroyed but he was ok. Thankfully, you were too.

I had a case failure out of my ria tac once. It was probably the scariest thing that has happened whit any firearm in my experience. The recoil was way different than any of the other rounds, i blew the follower, spring, and base plate straight out of the mag well at high speeds. I was very fortunate not to be cupping the butt of the gun with my off hand as im sure this would have messed me up. It was from an old batch of reloads my now deceased grandpa had loaded, which i dont shoot anymore. Glad you are okay OP check your weapon for damage and get back out there.
