I wonder how many actual incidents have occurred?... I just did a quick Google search and see several results - but didn't read 'em all, so I'm not sure if it's the same couple stories over and over and lots of reported incidents?...

      anyone know for sure?

ETA: I've had a Shield 9 for well over a year (maybe 2 now, I can't remember)... I don't shoot it as much as some other guns - but needless to say, haven't had any issues.

If by a few years ago you mean 15+, yes. The early gen1-2, chambers were not very well supported. That was corrected a long time ago.
There are numerous reason a gun will kaboom, even with an unsupported chamber it's still rare for a kaboom to happen if the ammo is in spec.

Does anyone know if there is a list of serial numbers affected by this?

That would be my first guess too. Mine is not flat on one end too, but I haven't had a problem getting it to lock back manually, it always locks back on the last round and no issues cycling. Initially it WAS stiff to get it to do so, but racking and use seem to help it somewhat.

Does not matter if the spring is strong or not. The trigger at least on mine and should be the case for all Shields, will not release the striker until the barrel is in full lock up.

I wonder why there's no photo of the barrel included with that? The slide looks ok, the innards are intact.. Was it a squib load or case head separation venting gas into an enclosed space? Most KBs I've seen split the chamber open like a log...

previous post said S&W is "denying warranty on the Kabooms"... so at this time no.

I'm assuming you are pulling the slide back ~1" to check. If so it makes sense the slide doesn't fully return as the slide is meant to ride home from a complete rearward position. That is nothing weird or out of the ordinary. If I remember correctly it actually happens with MANY handguns.

99.99999999999% of kabooms will be the result of bad ammo. Can happen with any gun regardless of brand or make.

Not saying there is not an issue with the Shield, just saying it seems a little early in the game to jump to that conclusion.

It is human nature, especially for the person who just had this happen to them, to blame the gun rather than their choice of cheap ammo or improper reloads. If it is is good brand name ammo, you still have to pay attention when loading your magazine and spot any defects that stand out. Like bulges, fractures, differences in length (pressed too far in or not enough). Any abnormality in the round, toss it. Better safe than sorry.
