Is this malfunction possible?

    I shot the FNP40 today, put about 85 rounds of FMJ 180gr CCI Brass ammo through it.

    I noticed on 2 mags the slide did not lock back on empty, it slammed forward and the hammer was now up (in DA mode) while I was shooting in SA. I wonder what could cause this?
