Assuming you are right handed...
With your left hand, wrap it over the top slide just in front of the rear sight (do NOT put your hand over the muzzle). Grip HARD.
With your right hand, position it 8-12" behind the grip of the gun. Now, in a movement like you are going to grab the gun's grip, ram your right hand hard into the back of the grip.
Since you are holding the slide solidly, this will drive the grip/frame forward rather violently. Hopefully, this will force the chamber open so that the live round is extracted. This technique allows substantially more force to be applied to the extraction process than gripping the gun and pulling the slide back, but the key is having a good, hard grip with that left (or support) hand.
Again, do NOT put your hand over the muzzle and do not touch the trigger during this process. Also make sure that the muzzle is pointing in a safe direction when you do this.
