Trigger control. Slow surprise as I like to call it then the groups will tighten up. Just keep shooting and it doesn't matter what others think about your grouping. Keep working and it'll come around. Enjoy.

this is for one hand shooting only, the pie chart people posted above is for one hand shooter.

two hand shooting will have the same basic hold as the one hand with the exception of adding a support hand for extra stability to reduce the minimum arc of movement.

That's interesting. This chart was given to me by a pistol instructor because of my tendency to shoot up and to the left with my G17 using a two-handed grip. I do not have this problem with my S&W revolver, nor did I with another student's 1911-type pistol (don't remember the brand). Is there an equivalent chart for two-handed grips?

I have found that dry-fire practice has helped, but has not completely corrected the problem. Especially when my heart rate is elevated...

This diagram just never helped me. I was shooting basically a vertical strip. Azimuth was fine - but elevation was either POI or in a line of shots lower.

Then realized it was some breathing issues, and some was flinching, and some was slapping the trigger (which is a poor term). Snapcaps helped a little, but basically gripping the gun with a more secure grip (depending on the gun) solved most of my issues.
