I've had that happen to me on a few of my USPs. It might be that your extractor is sticking a bit. You might just need a drop of lube in the right place. Take off your slide. On the top of the slide above the extractor there's a hole. That hole is where the roll pin to hold the extractor in place goes into. Place a few drops of lube in there and then work the extractor a few times to spread the lube around.

That...and one drop of lube on your extractor where it grips the rim of the brass. The finish can cause a tighter grip on the brass, thus FTE.

I have had my HK USP 9mm since 1997 and put my life on the line with it at one point. The only stove pipe I had with it was on purpose, while in class, when we were drilling to clear stove pipes & double feeds. Mine will eat any ammo I give it...Old, new, once fired, reloads, manufactured.

happen to my usp compact 9 first shot i fired but after that i had no problems.

With my old USP, I've only ever had 1 issue and it was a FTF at about 500 rounds. It was ammo related though, as most of the brass from that batch we had was severely bulged and wouldn't load at all through the other pistols we had on site. You mentioned different ammo, so I'd rule that out. You said you lubed it well, but I'd just want to make sure that the extractor area and punch hole above it has a drop mixed and worked in, while checking the extractor for burrs. Are your mags labeled? It'd be nice to know if it's only happening to one or another to help simplify the problem. Last thing, I never could get my USP to fail by limp wristing it. I'd basically hold it flat in the palm of my hand and squeeze off a round with another hand and it'd chamber a blank successfully.

You spent a ton of money on the gun, but shoot the cheapest ammo? Try other ammo. Watch your thumbs dragging on the slide.
