Another cause could have been high primers- recoil from the first round could have caused enough impact on high primers to set the adjacent rounds off. You can see the round on the left still has the bullet stuck between the cylinder and the frame. It looks like as the cylinder blew, those right and left rounds vented the gas upwards and the bullet didn't get much propulsion forward. This is so catastrophic that I'd almost have to say they used a charge of the wrong powder, probably a very fast powder like Hodgdon Titegroup, something with a very compact charge density, high energy, and fast burning. Destroying a gun like this takes more than just a few small errors in loading.
