Other than accidentally hitting the slide catch up with my thumb while firing, What would cause the slide to lock back with rounds still in the mag?

Location, location, location...of your thumb. Or, the slide lock was installed incorrectly.
One way to test is to pull the slide off the frame and see if the slide lock pops up on it's own. It should fall under gravity.

I've had it happen to me with hot reloads and some factory ammo. Just watch which ammo you shoot if you're positive you aren't riding the slide catchComment
