    Have you detail stripped it and taken a look at the safety components and the plunger tube yet?

      You're right - I didn't see the 3rd photo. I'm surprised actuating that way doesn't cause the left side safety to work it's way out and the next post would be "was operating my 1911 safety and all of a sudden the plunger went flying ....."

      I'd say the fix is to move the plunger forward in the tube then "pinch" the thumb safety from both sides and that should seat the tongue & groove arrangement. The left side worked it's way out rather than the right because the right side has that little wing under the grip holding it in.

      Sooner or later SA is going to put their Customer Service crew in charge of initial QC and these things won't happen as much -

        When the tongue and groove gets loose then wears, the ends of the fingers that make the groove typically spread.
        When the groove spreads, the worn fingers act like ramps and push the left side outwards (assuming the right side is trapped).

          I recently purchased a Kimber Custom II and had a gun smith add a Wilson Combat ambi safety since I am "wrong handed". The good news was he said it dropped right in, the bad news is it is also dropping right out - the right side safety works itself loose while shooting. Anyone know a gun smith with experience with this issue who can fix this issue? Thanks

            Finally fixed the issue. I sent it to Springfield and they said that the safety was worn out and that is why it would work itself out. They replaced and fitted the new safety for free. Awesome customer service!

            But for your case have a competent gunsmith take a look at the safety and see if the grooves are giving out. Just my .02

                    All of those guys specializes in 1911s with ambi safety. If your safety is just a little loose they can tighten it back up. Jimmy is a very good welder and machinist that knows metallurgy. He can tell if it's fatigued to the junk pile or can be saved.

                        I know OP got this sorted out, but when I detail stripped my brand new MC Op, with the slide off and I was re assembling everything, the safety did as OP's did. It had two distinct audible clicks, but when everything was re assembled, it worked just it should. Just a possible answer for others if they have this issue as well.

                        I thought of this thread when it happened to me.
