What the what?
The signature is supposed to be the person certified to roll, did he start the process and then realize his ink pad was broken or something? Did he charge you for his signature?
The UPS store I went to, the first thing the employee did was pick up the cards and swipe highlighter on what she wanted me to fill out. The instructions specifically said not to make any marks on the form (other than the required writins' of course). I left, emailed the AZ folks from home and they said as long as any marks didn't interfere with the prints, it was good.
I also wanted to avoid dealing with the local PD. There is the possibility that I would run into somebody who thought CCWs are a good idea and it was perfectly legal to get one from AZ and was ready willing and able to help, but then there's also a chance that Monica Bellucci will decide she's madly in love with me.
Anyway, they charged $20 per card, which isn't bad compared to my local PD, who charges $32 for the scan, plus extra for printing cards.
