Mark lll 22/45 trigger problem

My mark lll 22/45 is acting weird i have cleaned it and put it together many times but now when i load a mag, rack the bolt and pull the trigger it wont fire but if i pull on the trigger the second time it will fire and continue to fire. checked everything and cant seem to find whats wrong and the problem seems like its getting worse.
