You're actually looking for an illustrator with print design skills. Level of quality will definitely change the cost. A US professional will likely charge you at least $1k for your request. Fiverr might find you someone (likely overseas) that will do it cheaper.

Im a freelance Graphic Designer with some industry resources for custom illustrated work.
If the design is "simple" as you mentioned and can be done in house, i can most likely turn it around in a few hours in the format needed for your local tshirt printing shop.
My specialty is commercial logo design but making spoofs of logos was definitely one of the more fun parts of my previous training, and ive created some personal "spoof" designs for stickers and pvc patches.
