Default Question on the Intrafamilial form?

Hi,
My grandfather has a number of guns that he is looking to transfer to me (his grandson). All of the guns are "ok" (not assault, etc) as far as i can tell so i believe the Intrafamilial form (oplaw form BOF 4544A?) is the correct one to complete. I just had a question on how to complete this form. For the "owner information" section, does he, the current owner, complete that in his name? Or do I, the future owner, complete it in my name? Also, for the date acquired/acquired from section, do i need to put in when/how he acquired the gun or when he transferred it to me? In the declaration section, do I sign that or does he?

The other thing I am concerned about is that he is not sure of the exact acquisition date of most of the guns. Also, he indicated to me that some of them were gifts and were never formally registered (they date back to the 1950's). Is this a concern?

Also, I understand that you can attach multiple copies of the forms if more than 3 guns are being transferred, do you have to pay the $19 per submission or per form? (i.e. if i were submitting three forms would i pay $19 or would i pay $57?)

You are the owner. It is the date that you acquired them from Grandpa. Pay only $19 regardless of how many forms are used in order to register multiple firearms. It is not a concern that they were never registered, unless grandpa was to have registered any of them but I don't believe that is what you mean. You sign it. You must have a Firearms Safety Certificare or a CCW.

1. You are the owner, your information goes into that section.
2. You put the date you took possession of the guns.
3. The date's he acquired the guns does not matter. Only the date you did.
4. I'm 99% sure its $19 no matter how many forms you turn in at one time. Mail multiple form on multiple days, its multiple fees.

There is a spot in the owner's information section for your FSC or HSC number...that too will be your number, not Grandpa's.
