It was an ejection issue, but it was caused by the lack of purchase by the extractor. It started with the G3 9mm Glocks and carried on into the Gen4. The redesign of the Gen4 the recoil assembly was to address the frame flex issues if the .40 pistols...which initially caused feeding issues with the 9mm Gen4s

The problem is that the extractor doesn't maintain control of the fired case once it is extracted from the chamber. Since it is "floating" above the magazine, the ejector has no way to to kick it out of the ejection port (since there is no longer a pivot point). The case bounces against the breechface and then off the barrel hood.

This is easy to check on your 9mm Glock. Chamber a round, remove the magazine and fire the round. If the empty case falls out the bottom of the frame, the extractor wasn't holding it.

I will NEVER understand the obsession with many in the gun community to "tinker" with Glock pistols, thereby negating the single biggest selling feature of the guns... box stock reliability!!!

Switching it out for a stock extractor seems to have fixed the issue. Thanks to all you guys for the recommendations and education, really appreciate it.

Glad you got it sorted out. Can you do me a huge favor and take a picture of the SLB that was and presumably still is in the gun, and post it here?
