Some of the bigger things that jumped out at me:

Problem: they talk about instituting what amounts to affirmative action. That's illegal in CA.

No problem: the ban on FA setting on the M4 unless authorized. A friend of mine trained SWAT (not LAPD) some time ago, taught them not to use FA at all (there is no reason to use FA on a rifle in a police setting, really), but they always did as soon as they got a callout.

I've got a really big issue in their saying that SWAT caused the death of that child. They made a mistake, yes, but any deaths in that situation are on the BG. I'd be very surprised if SWAT didn't take every precaution to avoid what happened, and that's about all that can be expected.

I think that SWAT teams are often over used. Many times a simple surveillance team could grab the perp when he's taking out the trash. I watched a SWAT team raid a house once. They pulled up and did their thing. Closed down the street and everything. I had a good watch from my balcony. Unfortunately they didn't bother to see if the guy was home before. A stake out could have done that. Even a phone call. They threw in grenades and ran in there with their guns. No one was home. Afterwards when they were just hanging out at the scene one of the guys used the sledge hammer tool and smashed the fence gate to pieces. They also knocked down the front and back doors in the raid. What they didn't think about was the cost to the people involved. The house was a rental that had just been completely renovated. Owner had to pay for new windows, doors and a fence. All of that and the guy wasn't home. Lesson is that the SWAT was seriously mis used. A regular cop doing a stakeout picked up the guy that day.

I never understood why SWAT teams raid homes in this manner either. It seems that the one place a criminal would be armed and ready for battle would be their home.

I've done a little Bail Enforcement stuff in my short time here in AZ, and yea, it's just easier to get them when they come out, go to the grocery store, or get them right out of the car when they drive up. Makes things very easy when they're not in the home environment, it overwhelms them with unfamiliarity as we've sat there plotting our moves and how it will go down.
