Looks like a double feed.
Check the magazine lips...make sure they are not bent out of shape.

Need more information.

Was it the first round in the magazine and if so how was the slide released?

How many rounds did you fire and did the malfunction repeat? Was it confined to certain magazines?

Is this the first time shooting the pistol?

It happened on 4 different magazines, CMC Powermag, CMC Match grade, and 2 new Wilson Combat 47Ds. Towards the last couple rounds it would do this. I have a new 16lb recoil spring, ramp and breech are smooth. The only thing I can think of is extractor. I bought the gun used and the extractor is pretty worn.

you need to dremel out that ejection port 5cm.

How about 5mm instead?
5 cm = 50mm
25.4mm to an inch.
5cm = about 2 inches.

Normally I would think limp wristing or weak magazine spring since its happening when the magazine is near empty. However you have tried different magazines so something else is possibly wrong.

Next to magazines, the most likely cause of 1911 malfunctions is extractor tension and fit. Read up on it here http://www.m1911.org/technic2.htm and check your extractor tension.

Don't touch your ejection port. It's probably the ejector. Either it's all gummed up with crap and not grabbing the rim very well or it has too little tension.

That is know as a feedway stoppage, or failure to extract. The extractor has probably come out of proper tension. See the link listed above to correct!

Fix it and have fun!

I would have thought the same thing except that is a new round on top. It looks like the round poped out of the magazine before the slide closed. Since it is doing this with more than one magazzine it seems unlikely. Very strange, i have never seen that before.

yeah it's a double live round feed. I'm thinking the extractor is so weak/worn the rim on the loading round jumps out?
